This assessment is for BEP Research Study Project II purpose and based upon the following sources:

  • Review of BEP#08013 School 2008 I.E.P. report and school based assessment.
  • Three of progress questionnaires from Parent (8/30/08, 10/10/08, 11/26/08)
  • Interview with Parent (10/03/07/2008)
  • BEP Brain Function Test Repor

This evaluation report is focused on the improvement of the ability in brain functions, but no focuses on academic areas.

  • Does say words or approximations of words.
  • Does spontaneously request items, actions or activities.
  • Does ask for items or actions which may occur in the future.
  • Does aware of his surrendering.
  • He works well with adults.
  • Does engage in disruptive behavior.
  • Has trouble shifting attention from subject to subject.
  • Difficulty maintain with non-interested topic.
  • Has difficulties seeing options in situations.
  • Is able to remember/follow 2 steps instruction.
  • Does sometimes initiate greetings to others.
  • Does sometimes return greeting to others.
  • Is forgetful
  • Has difficulty keeping an organized area (room, desk, book bag, filing, etc.)
  • Has tendency to lose things.
  • Make careless mistake, poor attention detail
  • He demonstrates sensitivity to sounds and touching
  • Does engage in self-injurious behavior.
  • Is impulsive (doesn't think through comments or actions before he is said or done)
  • Interrupts or intrudes on others.
  • Difficulty with emotional control.
  • Has a tendency to hold grudges.
  • Does sometimes engage aggressive behavior towards others.
  • Does engage in repetitive speech and gross motor movements.
  • Has periods of rages behavior with little provocation
  • Has periods of dark thoughts, may involve suicidal thought
  • Appears anxiety caused by crowed, or the pressure from school works.

  • Increasing new words/sentences in his daily life.

  • Better control in speaking tone.
  • Improving in creativity and imagination from his cartoon painting production.

  • Difficulty following complex body movements and steps instruction.

Parents' Observation : March, 2009

Speech/Language/Communication Skills:
He can explain incidents better, and finish a 5 paragraph essay, also in communication area, he can make a longer conversation.

Sociability: He has better manner, more patience, he likes to make friends with more kids; he will say Hello more often

Sensory/Cognitive Awareness:
He has better balance, the times of falling reduced, he can memorize better,
He can take responsibility better. (forced less)

Behavior: He is much less with self injury behavior, better emotional control.
Temple tantrum reduced a lot.
He has more confidence and more positive thinking then before.

Health: He does not need medication for his allergy, and better control with his Asthma.
